Confidentiality Policy
OASIS
Family Crisis Centre, Inc. understands that community trust is among our most
important assets. We respect confidentiality, and we develop and uphold procedures
to ensure that private information stays private. Here's a summary of our confidentiality
policy as it relates to our programs.
OASIS
Family Crisis Centre, Inc. will protect our families information and will only
release this information when permitted or required by applicable federal and
state laws. No mailing list will be disclosed or sold for any reason. All information
obtained from client families will be treated as if it is our own. It will be
discussed only in private among our board members or volunteers and then only
in an effort to provide the best possible services and to make the best recommendations
available.
Information
will not be released regarding any drug or alcohol treatment, personal medical
facts, or conditions that we might be aware of except under any of the following
three conditions;
1)
The client family consents in writing;
2)
The disclosure is allowed by a court order; or
3)
The disclosure is made to medical personnel in a medical emergency, or to qualified
personnel for research, audit or program evaluation. OASIS FCC will strictly
follow Federal Regulations 42 CFR Part 2 regarding confidentiality of A/D records.
Any OASIS FCC Board member
who may have any OASIS information is to have it under a password only with
limited access at all times to those other members who have a need to know.
No one is allowed to obtain or have in possession any personal family files
or other confidential material without a valid need to have it. All family files
are to be considered CONFIDENTIAL.
